Homemade fairy costume/hair/makeup ideas?

Try your local craft store and get some wire, and bend it into the shape of wings, then cover them with a pretty fabric like tulle or chiffon. Make elastic loops into arm-bands to attach the wings. You could probably google a good pattern.
